Upon receipt of your request we will provide you with the information which we hold on you within 20 days of the payment being received.
1.4) Should you wish to retract your consent to be referred to our partner organisations, please submit your request in writing to the address above. Upon receipt of your request we will update your details to reflect this change within 20 days.

2.0) What is Get Healthy?
2.1) Get Healthy is a brand name of Parkwood Healthcare and we are commissioned to deliver lifestyle improvement services including referral hubs on behalf of local authorities. You are a client of Parkwood Healthcare and we are responsible for collecting and processing your personal information and details required to provide you with our services or to refer you on to our partner organisations.
2.2) Parkwood Healthcare as a business have responsibilities for looking after your personal information.
3) What information or data about you do we hold and why do we need it?
3.1) In order to provide you with advice and refer you to our partner organisations, we require your name, your address and your contact details.
3.2) We also require information with regard to your medical history and health in order to ensure that the Get Healthy program (and the programmes of our partner organisations) are safe and suitable for you. Get Healthy will only allow you to be referred to a lifestyle improvement service if it is appropriate and safe to do so, and we fully understand that this information is very personal and needs to be kept private.
3.3) When you were referred into Get Healthy either by yourself or by a health or social care professional, you consented to us using your information for the limited purposes explained and in order to provide you with an effective service. You will not be referred to any other organisation without your express consent which we will ask you for during your initial contact with Get Healthy.
3.4) If you were referred to Get Healthy on referral from your doctor or health professional we also require information relating to your ethnicity or any disabilities for the NHS's statistical purposes.
4) What do we do with your personal information?
4.1) Your personal information is only shared between the lifestyle improvement organisation you have consented to being referred to for the purposes of supporting you to attain a healthier lifestyle and Get Healthy.
4.2) We may also use the information to improve our products and services. We may periodically send promotional emails about new products, special offers or other information which we think you may find interesting using the email address which you have provided. From time to time, we may also use your information to contact you for market research purposes. We may contact you by email, phone, fax or mail. We may use the information to customise the website according to your interests.
4.4) If you have been referred to Get Healthy on referral from your doctor or health professional we have to share your anonymised information with your local authority as part of our duties under the referral scheme.
4.5) We do not share your data with any other outside organisation except with your express permission. We will not send you unrelated marketing material, nor will we transfer your information to third parties for marketing purposes.
5) Who is responsible for your personal data?
5.1) The organisation with principle responsibility for keeping your data safe is Parkwood Healthcare who can be contacted at the address shown in 1.2 of this policy.
6) Is your personal information kept securely?
6.1) We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place robust physical, electronic and managerial procedures to safeguard and secure the information we collect.
6.2) All data electronically transferred is encrypted and sent by secure methods, and data stored at Parkwood Healthcare is held on secure servers.
6.3) Any paper records are stored securely and destroyed when no longer required.
7) How long will information be kept for?
7.1) The information which you give us will be kept by Parkwood Healthcare for a period of 7 years after your last point of contact.
7.2) In addition we do keep data for statistical purposes which we use as part of our obligations to our local authority commissioners. This information is retained without specific reference to you and is therefore anonymous.
8) What should you do if you have a query about the accuracy of information we hold?
8.1) If you feel that any information which we may hold about you is incorrect then please let Get Healthy know and they will update our records.
